San Mateo County Health is seeking a well-qualified individual for the position of Complex Care Program Manager. This high-level manager will plan, coordinate, implement and manage activities of County-wide programs and services; recommend, develop, implement and evaluate goals, objectives, policies and procedures; provide leadership and coordination to community groups, public and private agencies, advisory boards and volunteers in planning and implementing programs; and provide highly complex staff support to senior level management.

There is currently one regular, full-time vacancy within the Public Health, Policy & Planning Division located in South San Francisco, CA. This position will travel/drive within the County to perform duties of the job. 

Oversight of complex care programs includes:

  • Bridges to Wellness:Program that provides care coordination services to individuals with complex needs including individuals experiencing multiple mental health and behavioral health co-morbidities, those who are unhoused or at risk of homelessness, and those who are at risk of avoidable hospitalizations and/or emergency department visits. 
  • Homes for Health: Program to place medically fragile individuals experiencing homelessness into subsidized housing. 
  • Housing for a Healthy California:Program designed to provide supportive housing opportunities for individuals experiencing homelessness or are recipients of or eligible for health care provided through the California Department of Health Care Services Medi-Cal program. This manager reports to the Director of Public Health, Policy & Planning and directly supervises seven staff and manages a team of 23 licensed and non-licensed staff.

Theprimary responsibilitiesinclude, but are not limited to, the following:  

  • Plan, direct, monitor and evaluate the effectiveness of activities, services and programs for the care of individuals with complex care needs. 
  • Direct, coordinate, and participate in the development and implementation of program goals, objectives, policies, procedures and priorities that balance both mandated services and public health needs.
  • Ensure services are coordinated with programs of other County Health divisions and County departments.
  • Maintain effective working relationships with a wide variety of public and private organizations such as internal and external county divisions and departments, external government agencies, community-based organizations and private contractors.
  • Conduct public relations to promote program goals, objectives and regulations.
  • Establish, facilitate and maintain effective communication, internally and externally.
  • Consult with management and medical staff on relevant program issues, develop alternative strategies for meeting public health needs and assist in the implementation of solutions as necessary.
  • Identify operational and service delivery problems and implement solutions.
  • Ensure client screening, monitoring, and tracking of service delivery.
  • Ensure proper coding and billing for services. 
  • Direct and assist in managing budget for programs and services.
  • Prepare, administer and monitor approved budget to ensure the accomplishment of objectives within budget restrictions.
  • Ensure that programs are funded with adequate resources to maintain mandated and expected levels of service.
  • Negotiate or participate in negotiation of contracts with outside vendors, consultants or organizations; monitor and evaluate related fiscal and legal liabilities.
  • Prepare and interpret administrative, clinical, financial, and technical reports, as necessary.

Through direct and indirect supervision, select, supervise, train and evaluate staff.

 

Ideal candidatewill possess:

  • At least three years’ experience overseeing programs or services for individuals with complex needs including those with substance use disorders, mental health challenges, chronic medical conditions as well as those that are unhoused utilizing a person-centered, recovery-oriented approach. 
  • Experience in managing programs with multiple funding streams, including federal, state, and local grants.
  • Experience implementing harm reduction principles throughout programmatic services.
  • Ability to implement change management principles and practices.
  • Experienced with analyzing and presenting data in ways that are easily understandable for a diverse array of audiences.
  • Experience developing solicitations for services; developing, implementing, and monitoring contracts; as well as grant administration. 
  • Ability to use critical thinking skills to draw upon knowledge base in order to address issues that arise within service delivery systems.
  • Experience with compliance with documentation standards, Short-Doyle and/or Enhanced Care Management (ECM) documentation preferred.
  • Experience with Microsoft Suite applications including Excel, Power BI, SharePoint.
  • Excellent written and oral communication skills.
  • Experience as a manager, coach, and leader with a track record of supporting programs and staff and motivating teams with humility to achieve equitable goals and objectives.
  • Experience working within a team consisting of licensed providers and non-licensed staff.
  • Supervisory experience of case/care management staff. 
  • Strong personnel management skills and ability to maintain a leadership position for multiple teams and disciplines.

Examples Of Duties:

Duties may include, but are not limited to, the following:  

  • Organize, supervise, and monitor specialized health-related services and activities.
  • Participate in the development of goals, objectives, policies, procedures and priorities within a specialized service area.
  • Prepare cost estimates and justifications for budget recommendations; monitor and control expenditures to ensure the accomplishment of objectives within an approved budget.
  • Assist in negotiating contracts with outside vendors, consultants or organizations; administer, monitor and evaluate contracts.
  • Maintain an effective working relationship with a variety of public and private organizations, including staff in other Health Services Agency Divisions, County departments and agencies.
  • Answer questions and provide information to the public; supervise the enforcement of community health-related standards, codes and regulations, as required.
  • Provide staff assistance to higher level management on program issues.
  • Select, supervise, train and evaluate professional, technical and clerical subordinates.
  • Perform related duties as assigned.

Licensure/Certification:

  • Depending on assignment, some positions may require specific technical licensure.
