$2500 Sign on bonus for full time field clinicians***

Central Coast Home Health & Hospice is a fast-growing, locally owned and operated organization with an excellent reputation throughout the Central Coast. We are now seeking a  full -time   Medical Social Worker to work with both  Home Health and Adult Hospice patients in the San Luis Obispo-Coast area (San Luis Obispo, Los Osos, Morro Bay, Cambria).  This individual will be working in our Hospice and Home Health Program with a lot of autonomy and flexibility while also receiving meaningful direction and supervision from an LCSW. Clinical Supervision is available.

Requirements:

• Bachelor's Degree in Sociology, Psychology, Human Development or master's degree from a school accredited by the Council on Social Work Education 
• Minimum of one-year social work experience in a healthcare setting, home health &/or hospice experience preferred.
• Understand the "hospice philosophy" and issues of death and dying
• Reliable vehicle to drive for work
• Current Driver's license
• Clean background & driving record
• Maintain auto insurance coverage
• CPR certified
